Purpose
The focus of the Care Management for At-Risk Children Program is to improve the health outcomes and reduce health care costs for children enrolled in this program. Case managers work with primary care physicians/medical home and the family to: 1) improve access to health care, 2) improve quality of health care and 3) decrease the cost of health care. Children covered by Medicaid are the primary target population of this program.
Availability
These services are provided by appointment Monday-Friday from 8:30am-5:00pm.
Eligibility
This service is for children, ages 0-5, who are on Medicaid and meet specific criteria. Eligible children are those who are at risk for development delay, have a disability or long term illness and/or social disorders.
